Baxter Recalls One Lot of 70% Dextrose Injection for Potential Stainless Steel Particles

What happened

On August 25, 2026, Baxter International voluntarily recalled one lot (Y495066) of 70% Dextrose Injection, USP, in 2000 mL VIAFLEX plastic containers because stainless steel particulate matter may be present in the solution. The pharmacy bulk package, not for direct infusion, was distributed from July 25-28, 2026, to healthcare providers and distributors in Alabama, Georgia, and Tennessee. Baxter said intravenous exposure could cause serious consequences, including vascular blockage, pulmonary embolism, organ damage, or death, and reported no related adverse events as of August 25, 2026. Customers were instructed to stop use, return affected units, and replace compounded solutions where applicable.

Why it matters

From a GMP/pharmaceutical manufacturing perspective, the potential presence of stainless steel particles in an intravenous product highlights the importance of controls intended to prevent and detect particulate contamination. The recall also shows why reliable lot identification, distribution records, and prompt segregation and return of affected units may matter when managing a product quality issue. Because the product may have been used for compounding, communication with downstream facilities and patients can be consequential.
